SOCIAL CARE COORDINATORS

Our primary work is administered through Social Care Coordinators (SCC). The Social Care Coordinators are highly skilled professionals who serve as a tremendous intervention; helping seniors, the unhoused, mental health and vulnerable populations overcome those SDOH’s that serve as barriers to quality healthcare.

Among the primary duties of the SCC’s include:

  • Set up a temporary computer for a Telehealth visit with a primary care physician or specialty doctor.

  • Assisting consumers complete on-line applications.

  • Assist consumers set up electronic equipment such as cell phones, computers, health monitoring devices, etc.

  • Teach consumers how to use their equipment.

  • Assist consumers in navigating on-line human and social systems to help them access services they need and qualify for. 

  • Medication Adherence Support.

HENM through our Social Care Coordinators will focus on reducing unplanned hospitalizations to use other types of acute care, increase the use of primary and non-acute care, increase the use of virtual physician visits, and increase the length of stay in housing by reducing transitions to long-term care facilities.
