Health Empowerment Network of Maryland, Inc. HENM offers a Sliding Fee Discount Program to make services more affordable for individuals and families with limited income.
Who is eligible?
The Sliding Fee Discount Program is available to all patients based on household income and family size, using the Federal Poverty Level (FPL) guidelines.
Discounts apply regardless of insurance status, including insured, underinsured, and uninsured patients.
No one is denied access to medically necessary services because of an inability to pay.
What services are covered?
The Sliding Fee Discount Program applies to HENM’s chronic care management and related services, including:
Chronic Care Management (CCM) for patients with two or more chronic conditions
Telehealth visits for chronic disease management and follow‑up
Remote patient monitoring review (such as blood pressure and blood glucose data review)
Medication review and reconciliation
Care coordination and navigation to primary care, specialists, and community resources
How the discount works
HENM uses a sliding scale based on household income:
At or below 100% of Federal Poverty Level (FPL): 100% discount – patient pays $0
101% to 200% of FPL: 75% discount – patient pays 25% of the actual cost
Above 200% of FPL: 50% discount – patient pays 50% of the actual cost
“Actual cost” is the standard charge HENM has established for each service, based on staffing, technology, and program costs needed to deliver high‑quality care.
Patients with insurance are responsible only for applicable copayments, coinsurance, or deductibles after insurance is applied. Uninsured patients may qualify for reduced charges under this Sliding Fee Discount Program.
How to apply
To request a Sliding Fee Discount, patients are asked to provide information about household size and income (for example, recent pay stubs, tax returns, or benefit statements).
